I finished the game right after it launched. I didn't realize there were optional bosses. But I did go back many years later, only to realize how much of mess it was.

Delbiters in Phantasy Star Online. The picture I found isn't that scary, but they are jerks. Being a force alone in Seabed alone is scary enough thank you very much.
 

Bloaters were kinda freaky but didn't bother me too much until my survivor playthrough. I only killed the 2 that you're required to kill and both just happened to die with literally the last round I had for any gun. Still remember saying out loud, "Fuck, my last bullet...I'm screwed" *fire* *bloater falls down* "...Thank God."
 
The Vortex aliens in Ecco the Dolphin. Partially because seriously look at those things, partially because of THE MACHINE

Re-Deads from Ocarina of Time still are an instant "nope" for me.
 
Disappointed to see no Ozma from Final Fantasy 9.

I saw a lot of Emerald Weapons from FF7, but always found the Ruby Weapon harder to beat.

The Giant Dragon in Mega Man 2 was another. It made me jump the first time and the controller fell out of my hands.

On that note, the Quick Man Stage in the same game. I always choose it last.

The Grim Reaper in Castlevania without Triple Shots.

More than one Eggplant Wizard in a room of all Kid Icarus Palaces.

Also, all of The Immortal... So much that I ended up getting something else.

Not sure how it is in Ultimate but in Tri I remember him being the equivalent of the boogeyman of high rank. You just never know when Jho will come around to wreck everyone.
In Ultimate the new Savage variant is even worse. The normal species' rage mode is Savage Jho's normal state, and you can imagine how much worse it gets when Savage Jho enters its actual rage mode.

oh, he still does that.

the image in that post is his powered-up version that appears very rarely in G rank. It starts off enraged and has a second level of rage, which looks exactly like the picture and is fucking terrifying.

at last, for christ's sake
i had the biggest NOPE moment of my gaming life during the LOTRO instance 16th Hall. While the rest of the team focused on the boss, minions and healing myself as an archer was given the easiest task ever: destroy some poisonous mushrooms which the boss used to regain power...I just had to shoot some arrows whenever one of them popped out. Things went smooth, group was a good group, boss' HP was about 3% when someone screwed up and a chain reaction of death was set in motion. Everyone dead but me and the boss, this huge pale monster who could have killed me in one second started to charge. Kiting fiesta! Slow traps like there's no tomorrow. It was the most suspenseful and fucking slowest final fight ever, it lasted about 45 minutes of me running around in circles, eventually shooting an arrow and the super slow monster trying to grab my arse
